Subject: FW: Transition Project for Youth with Special Health Care Needs

Dear All-

The DHHS Office on Disability (OD) (with support from MCHB and the HHS and HRSA's Office on Women's Health) are collaborating in an innovative project to improve transition health care services for youth with special health care needs. As part of this initiative, four discussion groups with male and female youth with special health care needs will be held to gain valuable insight into challenges and successes with transition services. The information garnered from these discussion groups will inform an Action Guide that will be distributed nationally and include recommendations and strategies to address challenges and key issues identified by the youth.

The project is currently looking for young men and women with special health care needs, ages 18 to 25 as potential discussion group participants.  The  contractor, New Editions Consulting, Inc., will organize and lead the groups.  Since many of you support youth advisory councils and/or youth partners, I thought you may know some youth who would be interested in participating.   Based on participant response and location details, discussion groups may be held in person or via teleconference or other electronic format. Participants will be compensated for their participation in the group. I've attached two files which provide additional information on the project.     

Ms. Lenhart from New Editions Consulting is willing to work with you to determine the most efficient and convenient means to distribute information to potential participants. You can submit a list of potential participants with their contact information, or distribute the attached youth information sheet through your organization's existing communication channels. Please feel free to share Ms. Lenhart's contact information with potential participants or other appropriate partner organizations.
