ALL THE ABOVE INFORMATION MUST BE PROVIDED TO PROCESS A REQUEST

The patient must live and must be treated in the U.S. The patient's
healthcare professional will be contacted to confirm a PKU diagnosis and
approve the product for their treatment. Product will be shipped UPS
ground
from the Nutricia North America offices in Maryland. Please allow up to
10
business days to obtain a consent and for product shipment.
Quantities are limited and available on a first-come, first-serve basis.
Requests must be received by April 15th, 2007.
