The New Clinical Report on Health Care Transition

Finally, planned proactive health care transitions are best practice for pediatric and adult health care settings!  The American Academy of Pediatrics, the American Academy of Family Physicians, and the American College of Physicians have just jointly published a new clinical report (Pediatrics, July 2011) describing what is needed for smooth transitions from pediatric to adult health care.

During the webinar, one of the guidelines’ lead authors, Dr. Carl Cooley, will walk attendees through this recently published report including the algorithm at its core.  Dr. Cooley and his co-presenters will look at the report from the perspectives of pediatricians, family physicians, internists, families and youth.  The report’s guidance will be linked to Got Transition’s three exciting learning collaboratives involving pediatric and adult health care settings in Washington DC, Boston, and Denver. The webinar will include a brief overview of their quality improvement initiatives.

Got Transition Learning Collaboratives Implement TransitionFamily Photo

Got Transition leads three practice-based Breakthrough Series-style Learning Collaboratives (LC) in Washington, DC, Denver and Boston and a fourth will soon be underway. In each of these LCs, five or more pediatric and adult, primary and specialty care practices are implementing, testing and evaluating a practical package of transition tools and systems for seamless transitions from pediatric to adult care.  This package, the Six Core Elements of Health Care Transition, mirrors the new clinical report appearing in the July 2011 issue of Pediatrics, "Supporting the Health Care Transition from Adolescence to Adulthood in the Medical Home."

Practices participating in the LCs each assemble a quality improvement team consisting of a lead physician, a care coordinator or other staff member, and at least one youth or family member. The teams participate in a 10-month quality improvement process to test and implement tools and processes for each of the Six Core Elements of HCT.
