During this session, participants can take part in an interactive discussion regarding Individualized Healthcare Plans (IHP’s). Presenters will share a Kansas project related to IHP’s and plans for expansion within the eight states of the Heartland Genetics and Newborn Screening Collaborative.


We will share with you our plans to develop a formal IHP process that focuses on a team-based approach to development and implementation of an IHP. We also hope to identify practices, protocols, and challenges Heartland states have used and encountered in the implementation and usage of IHP's in the school system. Additionally, we'd like to identify ways in which the medical and school community can be more collaborative in helping youth and their family’s plans for the future.


We will share about how one state in the Heartland region is striving towards standardized processes and practices around IHP's and what the Heartland region plans to do over the course of the next five years. Additionally, this time will be used as a means of gaining information from the Heartland states and engaging new partners across the region.

Objectives:
1. Share Kansas IHP initiatives to date.
2. Inform plans of Heartland Genetics Collaborative project.
3. Discuss IHP practices and activities in other states.
4. Gain interest for participation in an IHP learning collaborative.
