Subject: Apply for a Health Center Program Award!

Genetic Alliance invites proposals for the Family Health History Patient Education Toolkit: Health Center Program Awards. Genetic Alliance will select up to six HRSA-supported health centers, each to receive $40,000, to integrate the Does It Run In the Family? health history toolkit into their clinical care and/or outreach services.

Family health history is a simple, non-invasive way to capture information about risk. Implementation of these Program Awards will provide the foundation for an innovative, customizable strategy for integration of family history into diverse health centers nationwide.

Projects will be implemented over a 15-month period from May 2011 through July 2012. Project staff at each participating HRSA-supported center will customize the Does It Run In the Family? toolkit with personal health stories, photos, quotes, health condition information, and resources to make the booklets relevant to their communities. Centers will then integrate the customized versions of the toolkit into clinical care or outreach services. Centers will be expected to participate in a national evaluation and to provide feedback on how the toolkit can be implemented at other health centers and elsewhere in the future.

Proposals will be accepted until April 4, 2011. Genetic Alliance will consider proposals submitted from HRSA-supported health centers and any other organizations that plan to disseminate the toolkit in HRSA-supported health centers. A question and answer webinar will be held on March 3, 2011, from noon to 1:00 p.m. EST. Register online at http://www.geneticalliance.org/webinars<http://listserv.galists.org/t/544657/172173/97/0/>
